Physical Therapist (PT)** 15,000 Sign-on-Bonus!*The clinician hired for this position will be treating patients in the south valley.POSITION SUMMARY:You will help assist patients in restoring function, relieving pain, and preventing disability. You will provide preventive care, rehabilitation, and treatment for patients with chronic conditions, illnesses, surgery, or injuries while keeping detailed documents on patient progressRESPONSIBILITIES:Conduct an initial evaluation/assessment and report findings to the physician and Clinical Manager and develop the plan of care promptly for patientsPerform physical therapy procedures, measurements, evaluations, and diagnostic tests on patients in the homeDocument initial evaluation and treatment plan and re-evaluations of patients progress every thirty (30) days and update plan of care as necessaryComplete comprehensive assessments and OASIS data, as indicated on a timely basisAssist with orientation, supervision, and consultation services to Physical Therapy AssistantsObserve, record, and report to the Health Care Provider the patients treatment, response, and progress and promptly report significant changes and obtain orders to revise the plan of careCommunicate with the Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy servicesOther duties as assignedQUALIFICATIONS:Master's degree in Physical TherapyActive Physical Therapy license in the state of practice and in good standingAt least one year of experience working as a Physical TherapistMust have a vehicle, a valid current drivers license, and automobile insuranceAbility to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely mannerHome Health and Home Care Home Base documentation experienceBENEFITS (available to full-time employees):Excellent pay, including 23 days of PTO per yearHealth, dental, vision, 401k, life insurance, short-term and long-term disabilityContinuing education & tuition assistanceGenerous referral bonus programFlexible schedulingCompany Vehicle Program with a paid gas card for eligible full-time employeesABOUT US:Our unique focus is on patient-centered care and evidence-based medicine.
