JOB SUMMARYThe PCP Nurse Practitioner has the responsibility for
providing direct and indirect care to the participants in the PACE
Health Clinic and Day Center. - Works under the direct supervision
of the Medical Director/PCP. -Manages complex clinical situations
for which he/she is responsible. -POSITIONS SUPERVISEDNone
-ESSENTIAL DUTIES AND RESPONSIBILITIES
- Performs health histories, physical exams, and participant
teaching. -Orders, interprets, and follows up on lab tests and
results. -Performs selected procedures and documents care in
writing in EMR. -Initiates emergency measures as indicated.
-Recognizes abnormalities that justify immediate referral for
intervention by others.
- Initiates, evaluates, reviews and changes orders for selected
medications and diagnostic studies/treatments within attending
physician approved protocols.
- Assesses, initiates, and coordinates participant care on a
daily basis under the supervision of the attending physician. - - -
-Makes referral to other physicians including specialists with
staff PCP approval.
- Requests consultations from other services as needed in
collaboration with attending physician and/or established
protocols.
- Acts as liaison with physicians, nurses and other healthcare
professionals providing education, communication and support for
healthcare staff, participants, and families. -Directs participant
management in conjunction with physician staff.
- Participates in the PACE provider call schedule.
- Participates as a member of the Interdisciplinary Team. - - -
-Attends staff meetings and takes part in participant care
planning.
- Assists the IDT in formulating a plan for the coordination of
24-hour care.
- Documents participant changes appropriately in the medical
record and communicates participant changes to team members.
- Evaluates and treats participants in their homes, assisted
living facilities, nursing homes, and hospitals when medically
indicated.
- Precepts medical students and residents during their PACE
rotation.
- Encourages all participants to have Advanced Directives and
provides education as needed; may discuss the Do Not Resuscitate
(DNR) option with the participant and/or family members and may
complete the Health Care Wishes form and write DNR orders in the
medical record, in collaboration with a physician.
-REQUIREMENTS
- Master's degree in clinical nursing from accredited Nurse
practitioner program.
- 3-5 years nursing experience with Nurse Practitioner experience
preferred.
- RN licensure in perspective state and a Family certification
from Nurse Practitioner program.
- Only act within the scope of his or her authority to practice.

- Meet a standardized set of competencies for the specific
position description established by the state and approved by CMS
before working
- 1 year of experience working with a frail or elderly population
preferred. If this is not present, training on working with a frail
or elderly population will be provided upon hiring.
- Must have reliable transportation, a valid driver's license,
and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all
immunizations up-to-date before engaging in direct participant
contact.
- Pass a comprehensive criminal background check that may
include, but is not limited to, federal and state Medicare/Medicaid
exclusion lists, criminal history, education verification, license
verification, reference check, and drug screen. -SKILLS AND

geriatric participants; ability to get along with others in a team

decisions while reporting to PCP and Center Director.
