- AHA or ARC Only BLS certification required for onboarding
- Proof of vaccine and 1 booster or exemptions must be submitted
for approval
- Proof of 2 immunizations, positive IGG titer, if negative
titer, must have booster or Declination accepted for MMR
- Proof of 3 immunizations and positive antibody titer, if
negative titer, booster or Declination accepted for Hepatitis
B
- Required from October - March (unless otherwise indicated by
CDPH) Medical/Religious exemptions will be sent for approval with a
physician's statement(medical) or written statement from
traveler(religious) for Influenza Vaccine
- Must be within 30 days of start. Negative TB skin test,
QuantiFERON Gold or T-spot. Must state negative results. Updated
Annually. If Positive, must show proof of positive history, chest
x-ray within last 5 years and annual TB Questionnaire thereafter
for TB Test
- Statement signed and dated within 1 year of start stating
traveler is able to work without restrictions for Health
Statement
- Proof of 2 immunizations, positive titer or Declination
accepted for Varicella
- Proof of vaccination within past 10 years or Declination
accepted for Tdap
- Required on start date for clinical staff for Covid Test
- Resume or Profile required
- 2 Professional References, 1 minimum Supervisory required
- Must be within 30 days prior to start. 7-year County, National
Criminal Background Check, SSN Trace, Employment Verification(5
years), OFAC, and NSO. Updated every 3 years thereafter. If break
in service > 90 days, must run counties listed during break in
service for CHS Criminal Background Check Results
- Must be manual search within 30 days of start and updated
annually for CHS OIG Search
- Must be manual search within 30 days of start and updated
annually for CHS EPLS
- Must be manual search within 30 days of start and updated
annually for CHS OFAC
- Must be manual search within 30 days of start and updated
annually for CHS National Sex Offender
- CA State healthcare exclusion list. Medi-cal Program
eligibility completed within 30 days prior to the first assignment.
Must be updated with each extension for CHS Medi-Cal Suspended and

Updated annually. Rapid Drug screen accepted. Will accept within 40
days if the start date is postponed. If Break in service > 30

candidate along with date for CHS ROI for MyChart
- Attestation of any known record of DNS for the candidate from
prior 3 years on your agency's letterhead. To be signed and dated
by agency for CHS Any DNS (Do Not Rehire/Send)
- HLC Testing to be completed by Friday before Start. Candidates
Username and Password will be provided by CMC under the candidates
comments. If completed and confirmation provided Friday prior to
start, a $300 stipend will be paid for HLC Confirmation

Confidentiality of Receipt
- A diploma, degree or transcripts is acceptable. If applicable,
board certified, primary source verification of highest level of
education prior to first assignment and upon requalification for
